Special Edition CDI-Cast, The Basics of Clinical Documentation and Coding for Physicians Program Objectives

Program Description

Upon completing the session, participants will be able to:

  1. Demonstrate understanding of the relationship between physician documentation and the translation of that documentation into ICD-9-CM coded data 
  2. Demonstrate an understanding that ICD-9-CM coded data is used for planning,  reimbursement, quality ratings, Medicare Conditions of Participation, JCAHO Core Measures, and research.
  3. Provide documentation in the inpatient record that is timely, legible, complete, clear,unambiguous and precise. 
  4. Document detail and precision in the patient’s principal diagnosis
  5. Document all chronic co-existing secondary diagnoses
  6. Document all acute co-existing secondary diagnoses
  7. Document the clinical significance of all abnormal diagnostic tests
  8. Document the etiology or suspected etiology of symptoms

About the speakers:

Ruthann Russo has spent the past 20 years working with healthcare systems designing training programs to improve physician understanding of and compliance with clinical documentation, coding and billing regulations.  Dr. Russo has trained over 3,000 physicians in more than 200 healthcare systems throughout the nation.  She created the CAMP™ Method, used to train physicians to practice high quality clinical documentation. Ruthann is the former CEO of HP3, HP3 Research Institute, and Cabot Marsh Corporation. She earned her PhD in Business Administration from Touro University, her JD from American University School of Law, her MPH from Robert Wood Johnson School of Medicine/UMDNJ, and her BA from Dickinson College.  She is a registered health information technologist with the American Health Information Management Association. Ian Diener has spent the past 8 years as a physician educator and expert consultant to healthcare systems.  He has trained medical staffs in hospitals throughout the nation and spoken on behalf of several national trade associations. Prior to his work as a physician educator, Dr. Diener was a family practice physician for 20 years in suburban Philadelphia.  He was on the medical staff of Bryn Mawr Hospital, a part of the Main Line Health system in Philadelphia. Dr. Diener was also involved in the education of Bryn Mawr Hospital and Christiana Care Family Practice Residents and Thomas Jefferson University Medical Students.  He received his MBA degree at Lehigh University. .
